Transportation is provided to Optima Medicare Members who require transportation assistance to approved healthcare services. Southeastrans arranges transportation for eligible Optima Medicare Members residing in counties located throughout the state of Virginia.

Call Southeastrans to arrange non-emergency medical transportation services to covered medical services at least three (3) business days in advance of the appointment date. You can schedule up to 30 business days in advance.

Making Transportation Arrangements

When scheduling transportation, please have the following information ready for the Customer Service Representative when you call:

 

  • Your full name, phone number, address, date of birth, and county of residence
  • Your Subscriber ID as it appears on your Insurance card or your name & date of birth Specify any special needs (oxygen, escort) when you schedule the appointment
  • An emergency contact’s name and phone number
  • Destination information – address, phone number, doctor’s name, type of appointment, and name of the medical facility
  • Your mobility status (walking, wheelchair, or stretcher)

What is Non-Emergency Medical Transportation (NEMT)?

NEMT is a service that is provided to Optima Medicare members who require transportation assistance to Medicare-billable healthcare services and have no other means of transportation.

How do I apply for Non-Emergency Medical Transportation?

Contact the Medicare office in your county/region to apply for Medicare or to find out more about Non-Emergency Medical Transportation.

How is transportation determined?

Transportation is provided to Optima Medicare Members who require transportation assistance to approved healthcare services. Southeastrans arranges transportation for eligible Optima Medicare Members residing in counties located throughout the state of Virginia.

Call Southeastrans to arrange non-emergency medical transportation services to covered medical services at least three (3) business days in advance of the appointment date. You can schedule up to 30 business days in advance. Southeastrans arranges transportation for all Optima Medicare members residing in counties located in Virginia. Call the Southeastrans at 866-381-4860 (Optima Medicare Members).

Does Southeastrans provide private transportation to appointments?

Since this is a shared-ride service, it is possible that you will ride with other passengers picked up at or near your pick-up location. Southeastrans does not offer public transportation vouchers nor fuel/gas reimbursement.

Are there limits to how far I can travel to and from a medical appointment?

Transportation is available to services in the your home community. Travel outside your home community is available only when those specialized services are not available within your home community. Trips in excess of 50 miles require Case Management approval.   

Am I allowed to bring shopping bags on the vehicle?

You are only able to carry one bag onto the vehicle; bag must fit in your lap…no exceptions.

Am I allowed to take a friend or family member to appointments?

One adult escort or attendant may accompany a you if he or she has the same origin and destination as you and requires assistance for some medical reason. Escorts MUST be requested when scheduling the appointment.

When scheduling transportation, how much advanced notice is required?

To schedule transportation, you should book at least three (3) business days in advance of the appointment date. You may book up to 30 business days in advance.

Are members allowed to schedule transportation to and from non-medical appointments?

Optima Medicare Value, Optima Medicare Classic and Optima Community Complete members receive up to 24 one-way trips per member for non-medical appointments (in addition to standard supplemental benefit) to non-primarily health-related locations.

What type of information is required to schedule transportation to an appointment?
  • Your full name, phone number, address, date of birth, and county of residence
  • Your Medicare number as it appears on your Optima Medicare card or your social security number & date of birth
  • Specify any special needs (i.e. oxygen, escort, over-sized wheelchair, etc.) when you schedule the appointment

Southeastrans arranges transportation for all Georgia Medicaid Members residing in counties located in the North and Atlanta regions of the State. Call the Southeastrans number that corresponds to your county.

  • Atlanta Region Counties: Fulton, DeKalb, Gwinnett
  • North Region Counties: Banks, Barrow, Bartow, Catoosa, Chattooga, Cherokee, Cobb, Dade, Dawson, Douglas, Fannin, Floyd, Forsyth, Franklin, Gilmer, Gordon, Habersham, Hall, Haralson, Jackson, Lumpkin, Morgan, Murray, Paulding, Pickens, Polk, Rabun, Stephens, Towns, Union, Walker, Walton, White, Whitfield
